Good morning. Five stories today, and four of them come down to the question a court eventually asks anyway: what was written down, and when. We start in a theatre in Gomti Nagar on the morning of 11 September.

Nobody misidentifies a knee. So how does this keep happening?

The family says they found out only after it was over. She was discharged on 14 September. When they went back to ask how a left knee becomes a right one, they say the answer was pressure to drop it, then threats, from the doctor and the staff both.

The money has a problem of its own. Roughly ₹2.50 lakh went through the Ayushman card, and another ₹70,000 was deposited in cash. Treatment at an empanelled hospital under PM-JAY is meant to be cashless. A cash deposit sitting beside a card claim opens a second inquiry alongside the clinical one.

The CMO’s nodal officer has sent the hospital a notice demanding the operation record and the full treatment file. The SHO says what he knows came off social media, and that neither the family nor the hospital has filed anything at his station. For now this is an allegation and nothing more.

What makes wrong-side surgery so unnerving is that it is never a failure of recognition. It is settled hours earlier, in a consent form nobody reads twice, a list typed at 11pm, a patient wheeled in already asleep, a drape that goes on before anyone says “left” out loud. England made the WHO checklist compulsory in every theatre from February 2010. Between April 2012 and February 2020, NHS England still logged 460 orthopaedic never events.

WHY IT MATTERS
  1. Mark the site yourself, before the patient is sent for. It has to still be visible after draping, and rechecked if the patient is repositioned.

  2. Never mark the good limb “not this side”. The Universal Protocol marks one side only, the side you are opening.

  3. A signed checklist is not a completed check. The NHS audited compliance for eight years and still counted 460.

Six injections, one empty vial, ₹30 lakh: the negligence case that took eleven years to collapse

The patient was a lawyer with Meniere’s. He picked the vial up on his way out, read the name and looked it up: gentamicin, ototoxic, kills the nerves for hearing and balance. What had actually gone into his ear was dexamethasone. Gentamicin came up once, at the very end, for an ear that had already gone deaf. The commission found exactly one fault in the doctor, and nobody had sued him for it.

A normal 20-week scan, a newborn with half a spine, and an FIR only a court would order

Both legs were affected too. The father asked how a normal report had produced this, and says the doctors stepped away from the question. He went to the police and nothing was registered, so he petitioned a court. By the time a judge ordered the FIR at Vijaynagar station, the better part of a year had gone. Four people are named in it.

Delhi’s ₹600-crore procurement FIR is now a ₹750-crore case, with a fourth arrest

Rajiv Rangila left the country soon after the June FIR named him, and came back on his own. He landed in Delhi in the early hours of Thursday and surrendered to the Anti-Corruption Branch, which had him down as a proclaimed offender with a look-out circular against him. He is the first person held in this case who was not a government official. The agency calls him the prime beneficiary and says four companies he set up took the supply contracts. A court gave it four days of custody.

Salt iodisation worked. One child in ten now has antibodies against their own thyroid.

Doctors felt 8,903 necks between Srinagar and Trivandrum, the first national count in over a decade. Swellings are down from one child in four in 2003 to one in ten now, the visible kind has all but disappeared, and the old gap between richer and poorer children has closed completely. Then 5,746 of them gave blood. Antibody positivity was 4.4% at the last count, and the children carrying them are the ones the swellings are in.
