Good morning. Three of today's four stories run on paperwork: an indent slip, a fee notification, and a letter that went to eight government desks and came back empty. The fourth is a clock running in an ICU while everyone watches a different one. Air quality numbers at the end, because your OPD is about to feel them.

A strip bought at Rs 100 reached the government at up to Rs 1,00,000, officials say
The chain is Park Group of Hospitals, searched from 12 to 17 October along with a pharmacy company that officials say operated inside it. The mechanism they describe is a purchase price and a billing price that have nothing to do with each other. The hospital bought at cost plus about 20 percent and billed government health schemes at maximum retail price, and on these items the MRP ran anywhere from 10 to 1,000 times the cost.
Everything else feeds that gap. Patients covered by government schemes were sourced through doctors, agents and cash referrals. Outpatients were converted to inpatients and discharge was pushed back, because a longer admission means a bigger claim. Medicines left the in-house pharmacy against doctors' indents. Then, once the patient had gone home, nurses and billing staff reopened the file and added medicines, tests and quantities that were never given. In some files there was no patient at all. The trail runs back to 2020-21. No charges have been reported.
WHY IT MATTERS
The indent slip and the signature are the only parts of a scheme like this that would ever reach a consultant working honestly inside it.
The chart you sign at discharge is not always the chart the scheme pays against, so go and see what yours look like once billing is done with them.
Every figure above is a departmental estimate from a search that closed a week ago, so read all of it as allegation.
02 · EXAMS AND ADMISSIONS
The cap was Rs 3.5 lakh. Four colleges changed their label and now charge Rs 55 lakh a year.
A state committee decides what a private medical college in Tamil Nadu can charge for a clinical PG seat on the government quota. This year four private colleges became deemed universities, which puts them outside that committee's reach altogether. Their new clinical fees start at Rs 25 lakh. One year at the top end costs what sixteen years cost under the cap, and the state has gone to the Madras High Court calling the switch a use of legal loopholes.
The pattern is national. Twenty-three MD and MS courses at 12 deemed colleges now cross Rs 2 crore, and nearly two in five cross Rs 1 crore, up from one in three last year. Dermatology and radiodiagnosis head the list every year. Anatomy, physiology and biochemistry are free at several colleges. A rule letting the National Medical Commission cap fees on half these seats was written in 2022, and the colleges have kept it in court ever since. Round one allotment comes out today. The college-by-college tables, including the costliest PG seat in the country, are in the piece.

In Candida septic shock, 86% survived when the antifungal started within two hours. The usual wait was over 32.
Across 850 adults with culture-proven Candida septic shock at 32 hospitals over nearly thirty years, the clock ran from the first persistent fall in blood pressure to the first antifungal that covered the species grown. Past three days, fewer than 6 in 100 lived, and each hour in between cost about five survivors per hundred. These were units where antibiotics already arrive within two to three hours. The first regimen still missed Candida in nearly 6 of 10 patients, and across three decades that has not moved.
The hour-by-hour survival curve, the echinocandin-versus-azole split, and what the authors want changed.

Maharashtra pays an MBBS intern Rs 600 a day. On 2 November they stop work.
Rs 18,000 a month, out of which a Mumbai intern pays rent, food and the trip to work. They want Rs 30,000 and the promise in writing. Chhattisgarh, Uttar Pradesh, Uttarakhand and Jammu and Kashmir already pay Rs 25,000, leaving the Maharashtra intern Rs 7,000 a month behind. Letters went to eight desks, from the dean up to the chief minister's principal secretary, and came back empty. The state has until 1 November to answer. Emergencies will stay covered.
