Good morning. Eight stories that move Indian medicine, in your inbox by 6 AM with your morning chai. We open with a number that belongs on your extubation checklist rather than your induction one.

The GLP-1 risk sits at the end of the case, not the beginning

Across 119 NHS and HSC hospitals, 47,039 adults were screened before surgery. One in 36 was taking a GLP-1 receptor agonist — tirzepatide in 942, semaglutide in 353, and weight loss was the stated reason in 794.

Nineteen of those 1,348 patients regurgitated or aspirated around their anaesthetic. That is 1.41%, roughly one in 71, against 0.12% in everyone else — odds ratio 11.39. Two had gastric contents reach the lung. Nobody was re-intubated and nobody needed level 2 or 3 care.

What changes practice is not the size of the association. The confidence interval runs from six-fold to twenty-fold: definitely raised, loosely sized. It is where the events sat. Ten of the 19 happened at emergence. Gastric ultrasound, the one tool that tells you whether a stomach is actually full, was used in 17 patients out of 1,348.

WHY IT MATTERS
  1. Write the extubation plan with the care you give the induction plan. Ten of the 19 events, 52.6%, happened while the patient was waking up.

  2. Ask every patient by drug name. Two in five of these prescriptions came from neither the GP nor a hospital specialist, so the drug may never appear in the notes.

  3. Stopping the drug for a week buys you almost nothing. Tirzepatide's half-life is 5 days, semaglutide's is 7, and consensus guidance says continue.

Enzyme capsules failed the biggest test yet for pancreatitis pain

107 patients in Hyderabad, high-dose enzymes for 12 weeks, and pain fell just as far on a capsule of lactose and starch. The practice rested on a 19-patient study from 1983.

Finerenone approved for CKD in type 1 diabetes: first new drug in 30 years

The 34% that will get quoted in the corridor is not what the drug added. Placebo-corrected it is 25%, hyperkalaemia ran at 10.1% against 3.3%, and the label says lowers albuminuria rather than reduces risk.

A fabricated mouse study, delivered in seven and a half weeks

Undercover reporters ordered an experiment that was never performed. It arrived with 48 mice, a named laboratory and four figures. They asked for a bigger sample, and twelve more mice appeared. The provider's own description of who buys: young scientists and doctors under pressure to publish anything at all.

Resident duty hours: Supreme Court adjourns the 12-hour cap plea to 27 October

A 1992 notification caps resident duty at 12 hours a day and 48 a week. The petition says residents are worked up to 100. The only instrument carrying actual numbers is 34 years old, and PGMER-2023 dropped them.

Centre proposes CCTV in every pharmacy: what the draft Drugs Rules amendment says

Every Schedule H, H1 and X sale on tape, footage kept three months, objections open 30 days. The chemists' body puts the bill at ₹50,000 to ₹1 lakh a shop and wants it funded or dropped.

2,841%: Maharashtra's FDA opens the hospital consumables bill

An IV set bought at ₹11.05 and printed at ₹325. The private hospitals' body rejected a fixed markup, then conceded that routine consumables and complex devices should not be priced under the same logic.

NEET-SS cut-off cut to the 30th percentile: what it means for 1,857 vacant DM/MCh seats

Roughly 6,000 more candidates become eligible for one Special Stray Vacancy Round, about three per empty seat. The bar moved; the seat count did not. Nothing is live until MCC publishes.
